This program provides support in the home environment that would enable a person who is 60 years of age and older or a physically disabled adult over the age of 18 to remain in their home. Types of service may include homemaking, personal care, and home-delivered meals.
Program Eligibility: To be eligible for this program, the applicant must have difficulty caring for themselves or their home. You must be a resident of Tennessee and must meet limitation requirements for activities of daily living (i.e. bathing, dressing, maintaining continence, etc.) and instrumental activities of daily living (i.e. transportation, meal preparation, housework, managing medication, etc.). The Options for Community Living Program is considered the resource of last resort. If you are determined eligible for CHOICES or other programs, the Options program cannot provide services.
There is no income eligibility requirement for this program; however, there is a cost share, a sliding scale fee, if your income is above a certain level.
An AAAD Counselor is assigned to each consumer who has been approved for Home and Community Based Services. The AAAD Counselor does an assessment to determine needs, and if eligible, coordinates the needed services. These in-home services are provided by a network of providers already established and approved to work with GNRC’s Home and Community Based Services. The AAAD Counselor will continue to provide on-going assistance and coordination with eligible consumers. The Options program does not have the capacity to provide around-the-clock assistance. The amount of assistance is limited and will only provide what is needed for minimal support in the home.
